Case Management
JWCM's Case Management Service is primarily provided by Managing Director, Josie Welburn, who is experienced in arranging care packages and advising on equipment and transport needs for individuals who require specialist care.
We have a network of additional, specialist and support services we use locally to ensure the client receives a comprehensive rehabilitation package without delays occuring.
Personal Support and Training
Our Case Management service enables clients to access specialist, individual and quality care. By following the care expert’s report, the case manager can begin the recruitment of a skilled team of professionals to support the client.
The case manager will assist the client to arrange and manage the recruitment process from beginning to end; from advertising the position and attending interviews, to taking up references and ensuring that the selected staff are inducted and trained according to the client’s individual and specific needs.
Financial Support
At JWCM, our Case Management Service offers the client clear support and assistance with the financial aspects of care.
Organisiation and Liaison Support
Our case managers offer a client vital assistance with stabilising and maintaining supportive links and networks. These links can be with a range of people and services, from solicitors to local voluntary groups and may include the following:
At JWCM we specialise in the case management of adults and children with catastrophic injuries, such as brain and spinal cord injury, cerebral palsy, amputation and multiple injuries. We are able to offer our dedicated service to individuals not only within the Yorkshire and Humber region, but across the UK.
