Aug 2001 to date    MEDICAL CASE MANAGEMENT

Providing independent consultancy and case management services, immediate needs assessment, rehabilitation and nursing care cost reports to insurers and solicitors (claimant/pursuer, defendant and joint instruction), for which Bond Solon training in courtroom skills was completed.  Since January 2001, Josie Welburn has acted as an independent assessor under the Code of Best Practice on Rehabilitation, Carrying out immediate needs assessments on clients whose injuries ranged from minor whiplash to severe life changing injury such as, brain injury, spinal cord injury and amputations and other multiple injury cases. Many cases were also treated and supported for post traumatic stress disorder.

Apr 93 - Jul 01          Nurse Specialist in Physical Disabilities – Hull Community Health NHS Trust

Assessing care needs and providing care plans, health and social care support as part of a Community Care Plan for young adults aged 13-55.  Arranged and attended case reviews and statement reviews at schools.  Case managed complex physical disabilities. Provided support and counselling to the person with a disability and their family.  Provided on-going healthcare assessments for the provision of disability equipment, and disability awareness training for clinicians.  Involvement in developing care pathways for patients with motor neurone disease.

Jan 91 - Mar 93        Team Co-ordinator – Hull West Community Health NHS Trust

Managed a team of district nurses while retaining some clinical responsibility.  Involved in researching and development of a district nurse management tool to measure dependency of patients to identify workload of the nurse. Undertook teaching activities for RGN and DN students and provided training for healthcare workers leading to NVQ qualifications.

Jan 90 – Dec 91         Research Nurse – National Demonstration Centre in Rehabilitation, Hull

Seconded to undertake research on admission and discharge of stroke patients of all the Hull hospitals and subsequent service and support provision.  Developed project management and report writing skills.

Sept 85 - Dec 89       District Nurse – Hull Health Authority

I worked for a group of GPs and had 24-hour responsibility for the patients in my caseload.  Involved in managing care and providing nursing treatments to people in their own homes and residential homes.  My caseload comprised the elderly and those with chronic conditions/disability and the terminally ill.  Provided assistance to the paediatric DN.  Involved in assessment of needs, services and aids, and developing care plans.  This experience made me aware of the costs of supplying health and social care.

Jan 83 - Aug 84        Community Staff Nurse – Hull Health Authority

Provided nursing treatment to adults and children in their own homes, as prescribed by the DN.  This experience helped in the transference of hospital nursing skills into community nursing skills.

Gained experience working in the acute hospital setting, caring for adults and children with a wide variety of conditions including neurological, orthopaedic, surgical, medical, plastic reconstruction, ENT, and paediatrics.
