MEDICAL CASE MANAGEMENT
At JWCM our approach to case management is focused on complex injuries or conditions that require face-to-face work with the client, their treatment providers and/or employers.Our case management services are accessed directly through customer referral and are based on the severity of an individual’s situation and the barriers in relation to rehabilitation. Generally, the process will begin with an in-depth home assessment visit conducted by a highly experienced Case Manager, which will focus on the injured person’s current physical/functional status, treatment programme, psychosocial and environmental needs. A detailed Immediate Needs Assessment Report, describing the current situation based on observation and input from the client, is provided to pertinent treatment providers. This assessment report will include a rehabilitation plan outlining short and long term goals, and making recommendations that will aid the rehabilitation programme, interventions, barriers that may interfere with the overall goals, as well as estimated costs and timescales. |
